A Case of Subacute intestinal Obstruction

Case sheet:A 48year old male admitted in hospital due to abdominal  pain and constipation 

Chief complaint :patient complains of abdominal pain at umbilical region , constipation and abdominl distention since 1 month 

History of present illness:patient was apparently asymptomatic 1month back , suddenly he had abdominal pain at umbilical region with abdominal distension accompanied with gastric problems and constipation 
Type of Pain -dull aching type 
Pain is  Radiating to back 
Aggrevating factors are consumption of food 
Relieving factors - medication

History of past illness:
From past 2years patient suffering with irregular bowel movement and constipation which is rarely associated with abdominal pain.
Appearance of blood in stools 6months back 
Not a known case of asthma /Diabetes mellitus/hypertension .

Family history:Not significant

Personal history:
Diet:Mixed but vegetarian from past 3yrs but now patient is on fluids
Appetite: normal
Bowel.   :irregular 
Micturition:regular
Sleep.       :good 
Addiction.  :alcoholic from past 20yrs but not from past 2yrs
Drugs:tobacco chewing from 20yrs but not from 3months 

General examination:
Patient is conscious and moderate in built and moderately nourished 
Palor -No
Icterus-no
Cyanosis-no
Lymphadenopathy-no
Edema -no

Provisonal diagnosis -
Subacute intestinal 
obstruction
